2. Print clearly

3. This form must be signed by the Board secretary AND the Pastor

4. The guidelines for electing delegates and reserve delegates are published in the Manual {Par 320.3}

5. Please mail completed form to the Ministry Centre.

6. Delegate Credential Forms are due on or before January 31, 2008
